  本研究の目的は、介護保険制度における混合介護導入の影響を、非営利と営利の事業者との関係とサービス利用者の個人責任のあり方の視点からすることである。2021年度の成果は、以下の3点である。 まず、2019年度末に実施した調査を通じて得られたデータの整理と解析を2020年度から行って論文にまとめ、学術誌に投稿して2021年度に掲載された。混合介護については、「知らない」もしくは「導入する予定はない」という事業者が大半であった。また、民営化政策では商業主義的競争の原理と管理主義的競争の原理の両者が機能し得るが、介護保険制度では後者の原理がドミナントであり、よって、非営利事業者の「営利化」も抑制されている様子がうかがわれた。 次に、上記の検討の過程で、少数ながらも個々の事業者レベルにおいて、混合介護をめぐる様々な工夫がなされている様子が把握された。同時に、そのような工夫を可能にするには柔軟な発想を持つ人材が不可欠なのだが、往々にして人材の定着率が低く、新しい取り組みも継続し得ない様子がうかがわれた。そこで、介護等の対人サービス事業に従事しながら異動をしたスタッフを対象とするインタビュー調査を実施した。得られた結果の検討は、2022年に着手する。 最後に、本研究は方法論として社会学領域の組織理論を用いているのだが、我が国では未だに周知されていない新しい理論である。本研究の副産物ながら、社会組織理論のテキストをまとめ、刊行した。
• A Study of the Actual Conditions and Support Issues for Women and Families in Unstable Living Conditions in Japan

  In Japan, women in a wide sense of homeless are virtually supported by welfare services other than measures against homelessness. In this study, a user survey of welfare facilities that protect those homeless women was conducted and analyzed. As a result, it was found that there was a strong link between women's loss of housing and mental illness, and that some of female homelessness repeatedly used these social institutions and psychiatric hospitals from time to time. On the other hand, there were some women who had problems in living in a group and building relationships, and there were a certain number of those who were not suitable for institutional support. Behind difficulty in their life is severe damage experience of abuse and violence from their families and partners in their childhood and after adulthood, suggesting the need to consider the trauma in their support.

  This study compared and analyzed the marketization reforms and accompanying changes in the governance of social welfare and care services in seven countries: The United Kingdom, Sweden, France, Italy, Taiwan, Korea, and Japan. The main findings of this study show that, first, the marketization reforms were associated with the diverse pathways of defamilialization in social policy and the reorganization of intergovernmental relations. Second, the typology of the quasi-markets proved effective for elucidating the different situations facing marketization reforms in these countries. Third, there were commonalities and differences among these countries in their policy responses to problems in the social welfare and care labor market associated with the marketization reforms.

  Local level administration of the Long Term Care Insurance program (LTCI) in Japan was examined to assess variances in program outcomes, based on official statistics and interviews with city officials and care managers. Data were analyzed focusing on endogenous and exogenous factors to the municipalities. There were three key findings. First, because the structure of LTCI is centralized, local authorities are highly regulated. Second, relationships between providers and local authorities were an important source of variances in LTCI implementation. These relationships are determined by the path-dependent nature of service provision in rural areas and the quantity of providers in urban area. Third, variation in implementation was greater when clear central policies were lacking. These results suggest that if local authorities are to play central roles in LTC policy, it is necessary to consider the limitations of highly centralized institutional structure, and the path dependent nature of service development in the community.

  要介護高齢者の在宅生活の限界点とその規定要因を明らかにすべく、東京都葛飾区および秋田県大館市の居住者を対象に、平成15年度、平成17年度に引き続き、1373組の要介護高齢者と家族主介護者の第3波追跡調査を平成19年度に行った。平成20年度はこの縦断データの解析と、研究者が直接インタビューを実施して得られた質的研究データについて分析を進め、今までに得られた結果をもとに協力者に向けたリーフレットを作成し送付した。また、第1波と第2波のデータを中心に、11章からなる研究成果の単行本の発刊を予定している。

  The aim of this study was to explore the structure of care experience among elderly care recipients and their caregivers living in the community. One thousand and fifty-seven and 522 people who were eligible for long-term care insurance were studied in June 2003 and followed in June 2005 in Katsushika, Tokyo and in Odate, Akita, respectively. Elders tended to live alone and daughters or in-home helpers took roles of caregivers more often in Katsushika than in Odate. At the 2-year follow-up study, 22.6% and 33.1 %, respectively, of elders either died or admitted to the long-term care institutions. Main caregivers of deceased cases tended to be wives or daughters-in-law, and survival analysis showed that death was associated with caregivers' lower morale state or higher care burden and lack of basic care such as combing or shaving. Institutionalization was more common in Odate. This may explain better health status and lower malnutrition incidence in Odate. Institutionalized elders utilized more short-stay services and revealed dementia-related behaviors more often. In Japan, people are apt to regard in-home care giving as care giving living together. This obsession may urge families to choose institutionalization. Case studies suggest that care giving under pretending no concern contributes to stable care-relationship.
